Client Agreement:
I, on behalf of myself and my child(ren), agree to only enter the tennis court under the supervision of my instructor for the duration of my lesson. I agree not to access the tennis court or any other amenities outside of the allocated lesson times offered by The ICM Tennis. Violation of these terms may result in removal from lessons. I agree not to use any other facilities such as basketball courts, pools, volleyball courts, soccer courts, workout gyms and track and field.
ICM Tennis Policies:
Lesson Length: I understand that tennis lessons are 60 minutes in duration, with 55 minutes dedicated to instruction. The remaining time is for feedback, equipment sanitization, and lesson preparation. Makeup Classes: Due to limited court availability, makeup classes will not be offered for missed lessons.
Schedule Changes:
Lesson times may be subject to change, and The ICM Tennis does not guarantee a permanent time slot. Cancelled Classes: The ICM Tennis reserves the right to cancel classes at its discretion, with efforts made to provide notice.
Makeup Classes:
Due to limited court time, we do not offer makeup classes for lessons missed. Similar to other activities such as soccer, swim lessons, or hockey, participants do not have the opportunity to make up missed class time.
Cancelled Classes / Class Changes:
Although we strive to avoid cancelling classes, unforeseen circumstances such as issues on the tennis court or inclement weather may require us to cancel. In such cases, we will schedule a makeup session for you.
ICM Tennis aims to maintain consistent scheduling and instructors. However, occasional scheduling or instructor changes may occur. We will make every effort to keep parents and students informed of any upcoming changes.
Payment Agreement:
I understand that there will be no refunds or credits once my monthly payment has been processed. I authorize The ICM Tennis to debit my account on the 20th of the month prior on a monthly basis for tennis lessons. I understand that I am responsible for providing valid payment information and updating it if necessary before tuition is due. If the monthly payment cannot be processed, your reserved space will no longer be held. I understand that I may withdraw myself or my child(ren) from lessons by giving The ICM Tennis at least thirty (30) days' written notice of cancellation by sending an email to info@icmtennis.com. All withdrawals must occur at the end of the month. For example, to withdraw for the end of July, written notice must be received before June 30.
Release of Liability:
I hereby acknowledge that in consideration of participating in and attending tennis lessons provided by The ICM Tennis, I release, remise, and forever discharge The ICM Tennis and its affiliated companies from all claims, damages, causes of action, known or unknown, arising from participation in the Program. I further agree to indemnify the Releasees from any claims arising out of my actions while participating in the Program. The Releasees shall not be responsible for any damages caused or loss/theft of personal property. I acknowledge the risks inherent in tennis and accept personal responsibility for those risks. I agree not to commence litigation against the Releasees based on any claim arising from participation in the tennis lessons.
Medical Emergency:
I authorize representatives of The ICM Tennis to provide medical treatment in case of a medical emergency during participation in programs or activities. I agree to pay all associated costs for medical care and transportation.
Signature Agreement:
I have disclosed any medical/health problems in the comment box or student information section. I have read the tennis court rules specific to this facility. I acknowledge that I fully understand the terms of the Release of Liability, Waiver of Possible Claims, and Assumption of Risk. I acknowledge signing the release freely and voluntarily, without duress. I confirm that I have reviewed the ICM Tennis policies and payment agreement, and I agree that all terms are reasonable and signed freely and voluntarily.
